What
is the MEMBERSHIP
CARD?
The
Membership Card (shown below) contains the name and access code of the member,
i.e. the patient. This card provides the attending doctor
the patient's vital medical records, especially during
medical emergencies. All the doctor has to do is to log
into our website www.ERMedic.com
and enter the name and access code (refer to sample below)
found on the ER+Medic®
Card to display the patient's medical records online.

Your
medical information is otherwise completely private and
secure. You will choose
a User ID and a Password which you can use to enter ER+Medic®
to update your medical records any time.
How Do I
Update MY
MEDICAL RECORDS?
You
can complete your personal medical record by logging in
using your chosen user ID and password, and then by filling
in the easy to use online form. When updating your record, you
may choose to do it yourself or ask your family physician
for assistance. You can update your record at any time. It
is recommended that you log on at least once a year to
update your records.
Is My INFORMATION
CONFIDENTIAL?
All
your information is kept in a secured database, which is
protected by unparalleled internet firewalls and
encryption. Routine strikes are made to ensure the
integrity of this system. All information received from
you will be treated as confidential and will not be
released to any other party.
What If
I LOSE
MY CARD OR FORGET MY PIN?
If
you lose your card, email info@ermedicusa.com
and we will immediately deactivate your current access
code and reissue a replacement card at no cost. If you
forget your PIN, email info@ermedicusa.com
and we will mail a replacement PIN to your home address
within five (5) business days.
Can Anyone Change MY
MEDICAL DETAILS?
Your
medical information cannot be changed using your access
code shown on your card, as this is a read-only
access. The individual would have to have your User ID and
Password which is not shown on your card.
Should I Sign On Even If
I Have NO
MEDICAL PROBLEMS?
This
website contains more than medical information. It also
contains your particulars, next-of-kin contacts, insurance
policy numbers and agent contacts. This information may be
required when you seek medical treatment when overseas,
and is therefore made available to the attending medical
staff in an emergency.
Do I Always
Have To Carry The CARD
WITH ME?
It
is recommended that you always carry the card with you for
peace of mind. The information on the card will assist doctors and medical staff in the event of an emergency.
